Foundations, Corporate Giving Programs, or Donor-Advised Funds

Private Foundations: refer to Part 1, Line 25 of your most recent 990. Public Foundations: refer to Part1, line 13 of your most recent 990. *If headquartered outside of Indiana, dues are based on philanthropic giving in Indiana.
