Hazelett Award for Leadership in Grantmaking Form

The Hazelett Award for Leadership in Grantmaking was created in honor and memory of Susie Hazelett, a leader in Indiana’s philanthropic community for many years before her death from cancer in 2004. The award was established to recognize the efforts of an individual who displays character and leadership in philanthropy, including:

  • A giving spirit displayed by mentoring and nurturing professional development in others
  • A giver of time, talent, and other resources
  • A strong professional with the highest of standards, as well as integrity and creativity
Nomination Process/Guidelines
  • Only grantmaking members of Indiana Philanthropy Alliance (IPA) can nominate staff, board, or volunteers of a member organization. 
  • Current IPA board members are ineligible for nomination.  
  • Complete the nomination form/narrative (narrative should not be more than two pages). You may also include two supporting documents such as letters of support, a resume, etc., that describe:
    • Commitment: The nominee’s length of service in the field
    • Mentoring leadership: The nominee’s demonstrated leadership in mentoring and nurturing professional development in others
    • Impact: Examples of how the nominee has given of their time, talent, and other resources
    • High standards/integrity/creativity: Examples of how the nominee has demonstrated these characteristics
Selection and Recognition Process

A review panel of grantmakers, composed of five volunteer members who represent geographic and membership diversity, will review all nominations and select the winner. The 2024 recipient will be honored at the IPA Conference on October 7, 2024.

Please complete and submit the form below, including contact information for two individuals who can serve as references for the nominee. References should have a comprehensive knowledge of the nominee’s background and accomplishments. References for award finalists may be contacted at the discretion of the Award Selection Committee.
